Specialists that see patients on doctor's request:

Cardiologist - can follow up cardiac patients (ECG onsite)

Endocrinologist (usually to start treatment for osteoporosis post-fracture)

Ortho surgeon: sees patients having difficulty complication or occasionally, post-op routine follow up for patients who have difficulty going to acute care.

Neurologist: assesses patients with poorly managed neurological comorbidities or signs/sx of neuro problems that have not been diagnosed (more common than you might think!)

Psychologist: limited services, but can see patients for grief counseling, depression, anxiety, alcohol and drug addiction...

Gerontologist: sees 3 patients weekly to assess cognition, polymedication, etc. and make recommendations.
