HPIMr K History of present illness: 90 y/o male who will be 91 on April 1st and who, in the last 7 years, has had many falls thought to be related to weakness in the left leg possibly caused by spinal stenosis. He lost consciousness in only one of those falls, in 2019, and was hospitalized at l’Hôpital Sacré-Coeur for one week for investigations. No cause for the fall was found. He also fell again in early 2020 and was admitted to St-Mary’s Hospital where an Aortic Abdominal Aneurism was discovered. On November 15th, 2020, this patient felt a sudden and important abdominal pain following a fall and was rapidly admitted once again at St-Mary’s were it was discovered that the aneurism had ruptured. He was then transferred to the Royal Victoria Hospital for surgery and then returned to St-Mary’s. On December 1st 2020, he was transferred to the Julius Richardson Hospital for rehabilitation but never recuperated his former level of autonomy and the decision was made that he be permanently transferred to Ste-Anne’s Veterans’ Hospital. |
