This activity was designed to help you practice navigating the different sections of a typical in-patient chart, what lives in each section, and where to find key information that you will need to get to know your patient and stay on top of what is going on with her case from a team perspective...

As you go, please complete scavenger hunt document (make a copy and hand in):

Prepare patient profile document/Construct day 1 treatment plan based on chart review. Note that you do not need to complete the data collection portion of the document since this has been completed by the site therapist. The same document is used for both (copy/rename doc and then hand in):
