History

You take a history from David. He tells you he has not noticed being unwell, although with hindsight has lacked energy for the last few months. He gets swollen ankles at the end of every day. He wakes twice most nights to pass urine. He denies any other specific symptoms.

When you ask about past medical history - David says he has known he has high blood pressure for several years. He was prescribed a tablet for this, but forgot to take it regularly. He doesn't have any other medical problems. He does not take any regular prescribed medications. His father died of a heart attack in his 70s and his mother is still alive, but has severe dementia.
