Mrs Larson and her son Harry come to the surgery. Harry is wearing a school uniform. 

'Pity it couldn't have been later in the day - he has his last Spanish lesson before the oral today' she started.

You ask Harry what the problem is. He replies, with multiple interuptions from the mother. You note the following:

  • 15 year old male
  • Previously well. One admission for appendicitis age 9. Not on any medication.
  • 2 weeks ago had sore throat lasting 2-3 days, with mild fever. Was prescribed antibiotic (red/yellow capsules), but was getting better already.
  • Noted small tender lump on left side of neck at that stage. Initially seemed to get a little smaller but now is bigger again. In the last few days another one seems to be developing lower down the neck.
  • He has one sister, age 19 years, at medical school. Parents both architects.
  • Went to Goa 6 months ago for family holiday. Had mild diarrhea and vomiting at the time.
