1. Silence RequestA senior resident on the medical teaching unit is responsible for the care of Mrs. Alice Parker, a 48 year old woman, who was admitted to hospital 2 days ago with confusion. Six months prior, she was treated for lung cancer. The latest test results show a new diagnosis of metastasis to the brain. With medication, her confusion is starting to clear.
Mr. Kevin Parker, her husband, was at the nursing station and was just told about his wife’s test results. In a private room, he requests that you do not tell Mrs. Parker the diagnosis, as ‘she wouldn’t be able to handle it’ and that ‘he will make the treatment decisions for her’. As a preceptor, you must assess the resident in this situation to assess the professionalism competency using an In-training Evaluation Report (ITER) which will then become part of this resident’s ePortfolio. 1.
